ABOUT THE PROGRAMSub‑Acute Crisis Stabilization (SACS) program provides short‑term, intensive behavioral health support in a residential setting to youth experiencing acute emotional or psychiatric crises. The program focuses on rapid de‑escalation, stabilization, and connection to ongoing care through a collaborative, trauma‑informed approach. Services include comprehensive assessment, individualized treatment planning, therapeutic interventions, and coordination with families, schools, and healthcare providers to ensure a safe and supported transition to the appropriate level of care.
ABOUTTHE POSITION
Duties & Responsibilities
- Participate as member of multi‑disciplinary treatment team
- Track and coordinate client intake, discharge, services, and referrals as applicable
- Provide assistance to team including coordination of treatment planning and providing daily triage
- Educate and link individuals to self‑help groups, natural support and other resources as needed
- Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payee, legal services, medical services, education and employment
- Maintain careful documentation of contacts and communication to maintain quality services; complete all required documentation in compliance with agency policy
- Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients
- Assist with scheduling and providing transportation to and from appointments
- Maintain documentation in compliance with agency and program standards
- Participate in staff meetings, supervision, training and other meetings as assigned
- Establish knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, to provide culturally sensitive and competent care
- Provide crisis intervention and support as clinically indicated
- Additional duties as required
- Education: Bachelor’s degree in human service‑related field required. Experience may be accepted in lieu of education for some positions.
- Experience: A minimum of two years’ related experience preferred.
- Licensure/Certification/Registration: Valid driver’s license required.
- Training: Complete any program specific training required. Complete required annual agency trainings. CPR/First‑Aid Certification may be required within 6 months of hire.
