Location Detail: Backus Hospital, 10309

Shift Detail: First shift 8:00 AM to 4:30 PM Monday – Friday, rotating weekends and holidays.

Job Summary

Performs the social work functions of bio‑psycho‑social assessment, short‑term crisis intervention, supportive counseling and resource referral. Provides intervention to assist patients/families to improve social and economic difficulties that may interfere with health and wellness. Facilitates discharge planning across the continuum of care.

Qualifications
  • Education: Bachelor’s degree in Social Work or a related human services field. Master’s degree preferred.
  • Experience: Previous social work or human services experience. Two (2) to three (3) years of health‑care experience preferred.
Knowledge,

Skills and Abilities
  • Ability to read and understand oral and written instructions and effectively communicate information.
  • Basic computer skills, including opening and reading email and basic internet use. Working knowledge of Windows‑based software (including Word processing, database and spreadsheet) for occasional performance of duties.
  • Excellent interpersonal skills necessary to communicate with departments, employees, physicians and managers. Ability to relate to people with diverse educational and cultural backgrounds.
  • Must be organized, flexible and a team player; able to manage time and meet deadlines in a hectic work environment.
  • Requires the ability to take the appropriate action in response to changes, circumstances or problems. Requires a high level of analytical ability to gather and interpret information from various sources and formulate and implement a viable plan for intervention.
