About Us: Generations Family Health Center, Inc. serves a thirty‑seven‑town rural region in eastern Connecticut with sites in Willimantic, Danielson, Norwich, and Putnam. We provide primary care services to all ages, including medical, dental, behavioral health, care coordination, and community outreach. Mobile programs and School Based Health Centers are also offered.
Position OverviewFull‑time; 40 hours per week. The Family Nurse Practitioner will provide a broad range of clinical primary care services for adult, pediatric, and gynecological patients, including routine and appropriate ambulatory care.
Key Responsibilities- Provide age‑appropriate care to patients served.
- Perform office diagnostic and therapeutic procedures.
- Conduct therapeutic procedures such as injections, immunizations, suturing, wound care, and infection management.
- Instruct and counsel patients on therapeutic regimens, growth and development, family planning, emotional problems, and health maintenance.
- Develop and implement patient management plans, record progress notes, and assist in continuity of care.
- Perform comprehensive physical examinations and compile patient medical data, including health history and exam results.
- M.S. or D.N.P. degree from an accredited nursing school.
- Board certification in family nurse practitioner or equivalent.
- Current licensure in the State of Connecticut or ability to obtain one.
- Valid Federal and State Controlled Substance license.
Base salary starting at $116,000+ (salary based on experience). Bilingual stipend for Spanish or Haitian Creole fluency upon proficiency assessment.
- Primarily day‑time schedule (8‑hour shifts Monday‑Friday) with one Saturday 4‑hour shift per month.
