An exciting opportunity has arisen for a highly motivated and innovative Nurse Practitioner to join the Norfolk Forensic Community Learning Disability (NFC-LD) team based at Little Plumstead Hospital. The successful candidate will work as an influential and integral member of a diverse multi-disciplinary Learning Disability and Forensic team, working across a range of community settings in Norfolk with adults with a Learning Disability supporting them to remain living safely in community settings and reduce the need for admission to inpatient services.
A key feature of the team is its close working relationship with the on-site medium secure inpatient Learning Disability service, the Broadland Clinic supporting the transition of service users with needs relating to their risk profile and forensic histories back into the community in a timelier manner. The post holder will have the unique opportunity to work across both inpatient and community services.,
The successful applicant will be an essential member of the multi-disciplinary team, working in collaboration with Medical, Psychology, Occupational Therapy, Speech, and language therapy in assessing the service user's needs, devising individualised care plans and interventions, and evaluating outcomes which promote the service user's needs and wishes throughout the process., To be a Registered Nurse for people with a Learning Disability.(RNLD)
To function as a lead professional to a caseload of people with Learning Disabilities and associated mental health needs, as allocated, taking responsibility for formulation of appropriate care plans. To observe and abide by the NMC The Code:
Professional Standards of practice and behaviour and all other NMC professional guidance. To meet the N.M.C. Revalidation requirements and CPD as a registered nurse. To work towards the implementation of all national and local policies concerning the care and treatment of people with learning disabilities. To assist in providing and maintaining the management structure of the team. To work as a community learning disability nurse with knowledge & skills in the nursing assessment, treatment, and rehabilitation of offenders with learning disabilities.
To work as a member of a specialist multi-disciplinary team (M.D.T.) within the LD and Forensics Service. To practise evidence-based nursing interventions for offenders with learning disabilities within their caseload. To work in liaison with service users, carers, other professionals, in collaboration with other services ensuring a needs-led service. To be involved with and share the responsibility for identifying and developing opportunities for Service Users with learning disabilities to be key to local and Trust wide service operation and development.
To share their expertise and knowledge with their colleagues in the wider HPFT assessment and treatment services. To promote and maintain good working relationships and effective communications / liaison with carers, team colleagues in HPFT, Health & Community Service and external agents. To work in close liaison with the Clinical Team Leader and other clinical & professional leads in delivering effective evidence-based practice to adults with learning disabilities requiring specialist forensic services.
To develop & maintain relationships with Forensic Mental Health services in respect to national drivers. To carry out health assessments and be competent in reporting back health assessments to the wider MDT. To act in a consultative role to external agencies in relations to clinical interventions, risk assessments & management plans. To maintain appropriate records according to NMC record keeping & HPFT Data Quality policy and guidance.
