AboutThe Role
We are seeking an experienced Band 6 or above Registered Mental Health Nurse to join a dedicated Hospital at Home service. This is a rewarding opportunity to provide high-quality,
one-to-one care for a 21-year-old male with Autism and Learning Disabilities within his own home. The successful candidate will be compassionate, confident working independently and experienced in supporting individuals with complex care requirements.
- Provide dedicated one-to-one nursing care within the patient's home
- Deliver safe, individualised and person-centred care
- Support the patient's Autism and Learning Disability needs
- Monitor physical and mental health and identify changes requiring clinical intervention
- Follow individualised care plans and clinical protocols
- Maintain accurate and up-to-date clinical documentation
- Build positive relationships with the patient, family and wider multidisciplinary team
- Promote dignity, independence, safety and wellbeing
- Work collaboratively with healthcare professionals to ensure continuity of care
- Current NMC Registration
- Band 6 or above nursing experience
- Previous experience supporting individuals with Autism and/or Learning Disabilities
- Experience delivering one-to-one patient care
- Excellent communication and interpersonal skills
- Ability to work independently while collaborating with a wider multidisciplinary team
