Warwickshire County Council is seeking a Social Worker for the Discharge to Assess Team. You will manage a caseload, travel across Warwickshire and work with adults, their families and carers to support timely discharges.
Role includes attending Continuing Health Care/Decision Support Tool (DSTs) meetings, recording robust assessments and care planning, and collaborating with a wide range of health and social care partners. DBS checks apply.
