Location:

Norwood, MA

Shift: Tuesday-Saturday 10:00am-6:00pm

Salary: $50,000 per year

We support adults with developmental and intellectual disabilities through creative programs and services. Responsibilities
  • Collaborate with an exceptional team to plan and engage in the advocacy and support of our individuals, attending regular meetings as necessary.
  • Assist individuals with tasks including banking, grocery shopping, budgeting, paying bills, planning vacations, and managing healthcare.
  • Participate in social activities such as eating out, watching movies, attending sporting events, and other community events for our individuals.
  • Support individuals’ employment, education, social growth, health and wellness, communication skills, community mobility, and volunteerism.
  • Provide case management support using electronic (Icentrix) and physical case records.
  • Write and implement Individual Support Plans (ISP) and complete all required documentation.
  • Maintain monthly and weekly progress notes for individual goals and objectives. Communicate consistently with individuals and ISP team members.
  • Perform other duties and assignments at our various locations based on team and individual needs.
Requirements and Qualifications
  • A Bachelor's degree or an associate degree.
  • Must pass fingerprint background check, CORI, DPPC, and driving background checks.
  • Valid driver’s license required.
  • Valid MAP certification preferred (we will provide training).
  • Must own a vehicle and be willing to commute locally.
  • Excellent verbal and written communication skills.
