Position: Registered Nurse / RN Patient Care Manager

Description

We are currently seeking a dedicated Registered Nurse (RN) Patient Care Manager. In this role, you will play a crucial part in overseeing patient care, ensuring high-quality services, and promoting positive health outcomes. You will be involved in a variety of projects aimed at improving patient care processes and enhancing overall healthcare delivery.

As an RN Patient Care Manager, your role is vital in coordinating patient care activities, collaborating with healthcare professionals, and implementing care plans. Responsibilities:
  • Supervise and mentor a team of home health nurses, certified nursing assistants , and other caregivers
  • Develop, implement, and evaluate patient care plans in collaboration with physicians and other healthcare providers.
  • Ensure compliance with healthcare regulations, standards, and protocols.
  • Conduct patient assessments, monitor vital signs, and administer medications as needed.
  • Coordinate patient admissions, transfers, and discharges efficiently.
  • Provide education and support to patients and their families regarding treatment plans and healthcare resources.
  • Participate in quality improvement initiatives to enhance patient care outcomes.
  • Maintain accurate and updated patient records and documentation.
Requirements:
  • Active Registered Nurse (RN) license in the state of practice.
  • Associate degree in Nursing (BSN preferred).
  • Prior experience in patient care management or a similar role.
  • Strong leadership, communication, and interpersonal skills.
  • Knowledge of healthcare regulations and best practices.
  • Ability to multitask, prioritize, and work effectively in a fast-paced environment.
  • Proficiency in electronic health records (EHR) systems.
