Registered Nurse - Home Health

As a Home Health RN, you’ll play a critical role in helping patients manage their health and maintain their independence in familiar surroundings. You’ll care for individuals recovering from surgery, illness, or injury, as well as those managing chronic conditions or requiring end-of-life support. This role involves assessing patients’ needs, developing personalized care plans, administering treatments, and collaborating with families and healthcare professionals to ensure the best possible outcomes.

• Current RN license
• BLS (other certifications as required by facility)
• Two years of recent Home Health RN experience
• Strong communication and adaptability skills

• Assess patient conditions and develop individualized care plans.
• Administer medications, monitor symptoms, and provide skilled nursing care.
• Perform wound care, IV therapy, and other medical treatments as needed.
• Educate patients and families on disease management, medication use, and care techniques.
• Monitor and document patient progress, updating care plans as necessary.
• Coordinate with physicians, therapists, and other healthcare providers to ensure seamless care.
• Provide emotional support and guidance to patients and their loved ones.
