Support Worker, Community Health

• Long Eaton Area – Outreach Services
• Full time – 39 Hours per Week
• £12.21 per hour

• Provide day‑to‑day support with personal care and daily living tasks
• Assist with cooking, cleaning, shopping and medication support
• Support hobbies and community activities; access the community to an activity of choice or to purchase weekly food shop
• Provide 1:1 support from 8am–10pm and sleep‑in support overnight (10pm–8am); sleep‑in shifts are not contracted for any employee
• Deliver support both at home and in the community, making every day varied and rewarding
• Follow detailed support plans for individuals who may have autism, learning disabilities, a forensic background and may display behaviours of concern

• 39 hours per week, with shifts including days, nights, evenings and weekends

• Criminal background
• Grooming behaviours
• Mental Health Disorders
• Personality disorder
• Associated challenging behaviour
• Honest, caring, reliable, compassionate, flexible, self‑motivated, a good communicator, passionate about helping others
• Experience supporting adults with learning disabilities and/or autism is desirable; full training will be provided
• Enhanced Adult DBS check required
