Experienced Social Worker - Adults CHC Team

Rate: £30 per hour
Working Pattern:
Hybrid - 2 days onsite

Eden Brown Synergy is currently recruiting an Experienced Social Worker to join the Adults Continuing Healthcare (CHC) Team in Nottingham. This is an excellent opportunity for an experienced practitioner with strong CHC knowledge to work within a supportive and well-structured service.

• Completing Continuing Healthcare (CHC) assessments in line with the NHS CHC Framework
• Undertaking robust assessments, care planning, and effective case management
• Working collaboratively within multi-disciplinary teams
• Applying the Care Act, safeguarding procedures, and risk management principles
• Maintaining accurate and timely records using relevant IT systems

• Proven experience in CHC assessments
• Strong working knowledge of the NHS Continuing Healthcare Framework
• Sound understanding of the Care Act, safeguarding, and risk management
• Excellent communication and teamwork skills
• Confident IT skills with accurate record-keeping
• SWE Registered

• Experience working within NHS or Local Authority CHC teams
• Knowledge of the Mental Capacity Act and DoLS
• Experience managing CHC disputes and appeals

• DBS check processed and paid for by Eden Brown Synergy
