Village Health Group is a thriving busy dispensing practice serving 27,000 patients. Basedover 4 sites we have a large multi-disciplinary team led by our GP Partners which comprises GPs, Nurses & HCAs as well as Clinical Pharmacists, NAs, tNAs & Social Prescribers as part of our wider PCN ARRS team. Our clinical team is supported by our administrative, secretarial & reception teams.

We have an exciting opportunity for a Clinical Pharmacist to join our practice team working alongside our PCN pharmacy team. You will be a prescriber or in the process of completing this and will work as part of the general practice team within their clinical competencies to provide expertise in clinical medicines management across a range of aspects such as day to day medication related queries, acute prescription requests, supporting hospital discharge prescribing arrangement sand follow up, actioning of MHRA alerts and targeted medication reviews whichmay be done over the telephone or face to face.

You will be expected to help in streamlining processes and developing protocols to allow for safer prescribing and to meet CQC expectations.

You will act asa specialist resource to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals as required.

Clinical supervision and mentorship will be provided by the GP Partners for the pharmacy team.

Main duties of the job

We are looking for candidates who are enthusiastic self-starters and who are adaptable tochange. You will have excellent communication skills and be proactive in approaching patients, carers, colleagues and other agencies as and when needed.

It is expected that you will be able to use your initiative to gather information and to use the resources that are available to you, such as prescribing guidelines. Being a team player is essential. A good level of competence in use of I.T, predominantly Systm One is required.

The successful candidate will work within a supportive practice team and the wider PCNpharmacy team in order to deliver safe and effective use of medicines by applying their clinical, pharmaceutical and technical skills to a variety oftasks within the medicines optimisation agenda.

This role ispivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice.

THE PURPOSE OF THE ROLE

General Practice is at the heart of the Healthand Social Care system and it is critical to have thriving, sustainable general practices that meet local peoples health needs. The Clinical Pharmacist is instrumental in supporting the practice clinically. This is a key role, intended to support the delivery and development of the pharmacist role in primary care and ensure that we continue to provide high quality, accessible and responsive services to both practices and patients.

KEY RESPONSIBILITIES

  • Acts within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians in general practice.
  • Works as part of a multidisciplinary team in a patient facing role.
  • Takes responsibility for areas of chronic disease management within the practice and undertakes clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Provides primary support to practice staff with regards to prescription and medication queries. (helps support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the practice)
  • Provides clinical leadership on medicines optimisation and quality improvement and manages some aspects of the quality and outcomes framework and enhanced services.
  • Ensures that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
  • The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
  • Demonstrates understanding of, and contributes to, the workplace vision.
  • Demonstrates ability to improve quality within limitations of service.
