Were looking for a Personal Wellbeing Mentor - someone with a rare mix of compassion, resilience, organisation, self-motivation and flexibility.
This is for the person who understands that rehabilitation isnt just about offering advice; its about rolling up your sleeves and pro-actively supporting someone on their journey. This is a demanding role; you'll be working in a challenging environment with people who have complex needs and who can be reluctant to engage. But when you help someone break the cycle and you see them take real steps toward transforming their life, its a great feeling.
About the Role:
You will support a caseload of service users that may be due to be released from prison, recently released from prison, serving a suspended sentence, or on community orders. Based out of local probation offices, you will be meeting with service users in their local community, to support them with accessing social groups, healthcare, and various appointments. You will work collaboratively with Personal Wellbeing advisors, probation, and other external services to support them on their journey.
You'll often be able to plan your own schedule, but you will need to be able to prioritise effectively - its a challenging role where no day will be the same. What Youll Be Doing:
Community-based support:
You will guide individuals as per their action plan, to help them reintegrate into the community. This could be through practical well-being activities, mental health support, GP and dentist registration, or accessing food and clothing. You will be responding to individual needs, being pro-active, and tailoring your support accordingly.
Collaboration:
Your intervention may be part of a wider support package so collaborating with internal and external colleagues is a key part of the role. Caseload management:
Youll manage multiple individuals, balancing their needs while keeping up with all the related admin. You will case-conference with advisors regularly. Quality Recording:
Keep thorough, up-to-date records of all your work on our IT systems, ensuring nothing falls through the cracks and progress is properly tracked. Community Outreach, Group & 121 Delivery:
Deliver wellbeing support in various settings, either through tailored support sessions, out and about in the community, or utilising set interventions and learning materials.
What Were Looking For :
Relationship Building:
Youre able to connect with people at all levels, building relationships with Service Users in a way thats genuine, promotes a sense of trust and is non-judgmental. You will pay particular attention to confidentiality and maintenance of boundaries. Alongside this you will be engaging with key stakeholders within Probation and external agencies to build a network of support services for Service Users.
IT and Administrative skills:
Keeping meticulous records of every interaction to measure client progress and ensure effective interventions. Organisation:
You can handle a busy caseload and stay on top of detailed records. Resilience:
This role can be challenging, and results may take time, but you are the kind of person who stays committed and sees things through. Flexibility:
Youll need to be comfortable with change, as referral flows change frequently.
