What you will do:
Provide specialist occupational therapy interventions to stroke patients within the community
Work collaboratively with the multidisciplinary team to develop and implement personalised rehabilitation plans
Assess patients’ functional abilities and design tailored interventions to promote independence
Conduct assessments in patients’ homes and community settings to ensure holistic care delivery
Educate patients and families on stroke recovery strategies to support long-term outcomes
Requirements:
HCPC registration as an Occupational Therapist
Experience working with stroke patients or within community rehabilitation services
Strong assessment, communication, and clinical reasoning skills
Ability to work independently within the community while contributing to a wider team
Commitment to delivering high-quality, patient-centred care
