Overview
Principle duties to include:
Contacting all patients newly listed for surgical procedure to ask or ensure they have completed a pre-screening questionnaire, and where required support them to complete the form.
- Collate information at the point of referral from primary care and in the pre-screening questionnaire.
- Use the above information to initially triage patients and identify those likely to require optimisation, such as those with known co-morbidities, poor physical fitness, nutrition and or high BMI.
- Information should then be compiled in a template suitable (or use available IT systems where available) for clinical review and sign off.
- Triaging will be supervised by a registered healthcare professional and no clinical decision making will be made by Care coordinator alone.
- Contact patients to inform them of outcome of pre-screening triage, and where appropriate and under the direction of a qualified health professional, use approved materials to provide them with universal advice about pre-operative preparation, including diet, exercise/activity and healthy living.
- Under the guidance of a registered health professional, arrange appropriate follow up appointments for higher risk patients.
- Develop and update a database of health promotion services available to patients in their local area, this will include those commissioned by Nottingham and Nottinghamshire ICB.
- Use this knowledge to refer patients to their most appropriate local services. Examples will include activity classes, weight loss and psychological support.
- Refer patients to NUH health promotion services, for example smoking cessation, alcohol reduction and diabetes control.
- Where relevant, encouraging patients to find out if their GP has a social prescribing link worker and/or health coach, and how they can refer themselves.
- Work with local ICB or healthcare system to identify community links and initiatives that may benefit patients.
- Caseload management of patients requiring further intervention and/or medical optimisation/assessment to ensure referrals to hospital medical specialties or support in the community have been picked up and acted on by receiving teams/services.
- Maintain a worklist of patients and send out automated messages every 3 months to check that health conditions have not changed and that the patient still wants to have the surgery.
- Notify clinicians of any changes in patients health status.
- Liaising with booking and scheduling teams about planned surgery dates and to keep them informed and aware of when a patient is optimised for surgery.
- Supporting monitoring, evaluation and quality improvement.
- Triaging will be supervised by a registered healthcare professional; no clinical decision making will be made by the Care coordinator alone.
- Ability to compile information into a template or use available IT systems for clinical review and sign off.
