Somerset NHS Foundation Trust is seeking a dynamic senior Occupational Therapist to join the Discharge to Assess (D2A) service as part of the wider Intermediate Care offering in Somerset. You will lead advanced assessments and discharge planning to support safe, timely hospital discharge.
You will assess patients in their usual residence, coordinate care across health and social care, and contribute to service development with evidence-based practice.
