Care Coordinator; Domiciliary

Firstcall 247 Limited is looking for a Care Coordinator to join our team at Nottingham branch.

You will be working with the team to ensure that the business and compliance met the target.

Key responsibilities:

• Allocating right skilled and experienced care workers to deliver the best care as per the needs of the service user.
• Ensure the service users are at the heart of care delivery.
• Organising the rota on the system.
• Playing a key role in getting new contracts.
• Ensuring the assessment is done and the care plan is finished on time.
• Must ensure the documentation is in situ and completed for each service user.
• Maintaining a strong relationship with the key stakeholders which includes Social workers, District nurses, Safeguarding team, commissioning teams, Occupational therapist and PTs.
• On‑call duties as and when required.
• To achieve targets, KPIs and objectives set by the Registered Manager.
• Ensure all staff meet the compliance as required by CQC.

We looking for :

• At least 2 years of experience as domiciliary care coordinator.
• Able to work as a team player and must have the do‑it mindset.
• Excellent organisational skills, communication skills both written and verbal.
• IT knowledge:
  Microsoft Office.
• Driving licence.
• Experience handling complaints and problem‑solving skills.
