Dorset Stroke and Neuro Service (West) is seeking an enthusiastic professional to join our community-based team in Dorset. You will support rehabilitation and care for individuals with stroke or neurological conditions in their homes, clinics, and group settings, including via technology.
You will work within a multidisciplinary team, perform assessments, and deliver six-month reviews, ensuring patient goals are met while adhering to health and safety and confidentiality policies.
