What you will do:

  • Conduct comprehensive assessments of patients recently discharged from hospital settings.
  • Develop and implement tailored rehabilitation plans to meet individual patient needs.
  • Collaborate with multidisciplinary teams to ensure seamless patient care and transition.
  • Provide training and support to junior staff and healthcare assistants.

Qualifications / Requirements:

  • Must be registered with the HCPC.
  • Possess a minimum of 2 years of experience in a related specialism.
  • A car driver is preferable to facilitate visits across locations.
