We are looking for an experienced Advanced Clinical Practitioner to join our friendly team. We are an Equal Opportunities Employer
with a practice list size of approx 16000 patients and are based across 2
sites.
Full-time or Part-time considered.
Salary:

Main duties of the jobThe successful candidate will provide expert, autonomous clinical care within a busy and supportive general practice, managing patients with a broad range of acute, urgent and long-term conditions. Working as an integral member of our multidisciplinary team, you will assess, diagnose, treat and, where appropriate, prescribe independently, ensuring patients receive timely, high-quality care.
You will play a key role in improving access to primary care by managing your own clinical workload, making evidence-based decisions and referring patients appropriately when required. In addition to direct patient care, you will contribute to health promotion, preventative care, clinical governance, quality improvement initiatives and service development.
Working closely with GPs, Practice Nurses, Clinical Pharmacists, First Contact Practitioners and other healthcare professionals, you will help deliver safe, effective and person-centred care while supporting the ongoing development of our clinical services.
We are looking for an experienced, compassionate and motivated Advanced Clinical Practitioner who enjoys working both independently and collaboratively and is committed to delivering excellent patient care, continuous learning and service improvement within a forward-thinking primary care team.

patients.

Job responsibilities
We operate a Total Triage system at the practice.
The ACP will make professionally autonomous
decisions for which he/she is accountable.
Provide a point of contact within the Practice for
patients presenting with undifferentiated, undiagnosed problems, making use of
skills in history taking, physical examination, problem-solving and clinical
decision-making, to establish a diagnosis and management plan Instigate necessary invasive and non-invasive
diagnostic tests or investigations and interpret findings/reports
Where the post-holder is an independent prescriber:
to prescribe safe, effective and appropriate medication as defined by current
legislative framework
Provide safe, evidence-based, cost-effective,
individualised patient care Offer a holistic service to patients and their
families, developing where appropriate an ongoing plan of care/support, with an
emphasis on prevention and self-care Promote health and wellbeing through the use of
health promotion, health education, screening and therapeutic communication
skills
Refer patients directly to other services/agencies
as appropriate
Work with the Practice lead in developing and
delivering specialist primary care services, such as long-term disease
management (Diabetes, Asthma, Chronic Obstructive Pulmonary Disease and
Hypertension)
Work with nursing, medical and health care
assistant colleagues to ensure that National Service Frameworks are being
delivered
