Nottingham City Care is pleased to offer an opportunity for a motivated individual to specialise as a long-term conditions Dietitian within our established, forward-thinking integrated Diabetes Team. We are looking for a team player who reflects City Cares values and behaviours and can contribute to our high-performing, highly skilled service. The successful candidate will play a key clinical role in providing person-centred assessment, care planning and coordinated support for people with diabetes across Nottingham City.
The role aims to improve engagement with appropriate services, support effective specialist diabetes management and reduce avoidable poor health outcomes, including unnecessary diabetes-related hospital admissions. Our multidisciplinary team works with Nottingham University Hospitals Trust and wider community services to deliver high-quality, community-based diabetes care for the people of Nottingham City. The team includes consultants, Diabetes Specialist Nurses, Advanced Diabetes Dietitian, Educators, Clinical Support Workers and Administrators.
duties of the job
We are looking for a dietitian who has experience of working with individuals living with diabetes, be able to manage and work collaboratively within the integrated diabetes team to ensure effective care for the people within Nottingham City. The post holder will be expected to support the development, planning and implementation of local service strategies whilst ensuring compliance to both local and national guidelines.
The Dietitian will need to demonstrate expertise within the long-term health condition, Diabetes using evidenced based practice to develop education and training to both people living with diabetes and other healthcare professionals.
This post is a clinical patient facing role which includes the delivery of complex Type 2 clinics, Type 1 multi-disciplinary clinics, domiciliary visits and structured education across various venues in the city. There will also be opportunity to support the advanced diabetes dietitian with the development of a community insulin pump service working in collaboration with Nottingham University Hospitals.
