Virtual Ward Community Practitioner

Job Summary

We are seeking an enthusiastic Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward! One full‑time position is available within a collaborative system‑wide Virtual Ward service across Nottinghamshire Healthcare, NUH, Sherwood Forest Hospitals, Doncaster & Bassetlaw Teaching Hospitals and City Care.

As a Band 6 Senior Community Practitioner you will provide timely assessments, person‑centred care, and advanced clinical decision‑making in patients' homes, working autonomously and within an established MDT.

• Deliver timely, person‑centred assessments and provide high‑quality clinical care to patients on the Virtual Ward for up to 14 days.
• Work autonomously and within the MDT (including Matrons, Nurses, AHPs and support staff) to prevent unnecessary hospital admissions and support safe early discharge.
• Interpret remote monitoring data, undertake face‑to‑face clinical reviews, identify deterioration and initiate appropriate interventions or escalation.
• Lead and coordinate safe, effective discharge planning and contribute to daily MDT discussions on treatment and risk.
• Support junior staff through supervision, role‑modelling and clinical guidance.
• Maintain accurate documentation on Systm One and uphold professional standards, governance, and patient safety.
• Work flexibly across respiratory, frailty and admission avoidance pathways, following lone‑worker and dynamic risk assessment procedures.

• Registered general nurse (Level
  1)
• Able to demonstrate management competencies
• Evidence of working at a senior level
• Degree level qualification or equivalent knowledge and experience in a health‑related subject (e.g., Specialist Practice Degree or Leadership Qualification)
• Advanced clinical assessment evidence (practical evidence or courses)
