Community Staff Nurse

Job Summary

We are looking for an experienced registered adult nurse to join our Community Nursing team on a 12‑month fixed‑term contract (Band
5). The role focuses on delivering skilled nursing care in home and clinic settings, with a particular emphasis on wound care, palliative care, continence care, and long‑term condition management.

• Provide skilled nursing care and support the team to deliver high‑quality integrated care.
• Facilitate integrated care following a holistic assessment, enabling patients to be cared for at home and in clinic settings.
• Prevent unnecessary hospital admission/re‑admission by reducing the effects of ill health and disability.
• Participate in a robust induction and competency‑based development package.
• Support a flexible schedule from 8am to 8pm, seven days a week, with reactive evening and night nursing coverage.

• Registered adult nurse with Band 5 competence.
• Excellent communication and interpersonal skills.
• Adaptable, reliable, resourceful and approachable.

Please note applicants will be required to pay for their DBS check. The cost (£26.40 standard or £54.40 enhanced) will be deducted from salary over the first two months. An annual DBS Update Service fee of £16 per year may apply.
