The core purpose of this post is to take responsibility for providing high‑quality, patient‑centred nursing care within a GP practice.
Key focus and skills: meet the nursing needs of patients accessing appointments at Greendale Medical Centre, including long‑term condition management, screening, preventative care, patient education, wound care, clinical audits, and supervision of less qualified clinical colleagues, while working in partnership with the Lead Nurse, GPs, management and administrative colleagues.

Responsibilities- Deliver a range of nursing care, including wound care, dressings, contraception, venepuncture and immunisations
- Carry out health checks and screening, including opportunistic interventions (MECC)
- Assess patients and respond to clinical needs
- Support patients with diabetes, asthma, COPD and hypertension; deliver planned care in line with protocols; work with the multidisciplinary team to manage patient care
- Provide clear advice on conditions and treatments; support self‑management and independence; deliver lifestyle advice including diet, exercise and smoking cessation
- Collaborate with the wider practice team, neighbourhood and community teams to support ongoing patient care and improved health outcomes
- Support development of Healthcare Assistants and junior staff; contribute to and promote a positive inclusive team culture
- Maintain accurate clinical records using appropriate agreed codes; complete all documentation in a timely manner
- Follow infection control procedures, including handling of specimens, spillage of bodily fluids, sharps injuries and disposal of waste materials
- Ensure mandatory training is up to date, including safeguarding and BLS
- Support audit and quality improvement activities; engage with ongoing QOF requirements and achieve quality indicators and targets
- Work in line with National policies and procedures, including NICE Guidelines, PGDs and PSDs; ensure daily checks, fridge temperatures, drug monitoring and stock checks are completed and uploaded to Team Net
- Maintain confidentiality of patient, staff and practice information in accordance with federations policies and procedures
- Registered Nurse (Adult) with current NMC registration
- Evidence of ongoing continuing professional development
- Primary care qualifications (e.g. diabetes, asthma, cytology) – desirable
- Teaching or mentorship qualification – desirable
- Strong clinical skills including wound care, venepuncture and immunisations, or willingness to undertake training
- Ability to assess patients and recognise deterioration
- Ability to support long‑term conditions including diabetes, asthma and hypertension or willingness to train
- Strong patient education and communication skills; promote self‑management and lifestyle change
- Ability to manage workload and work under pressure; accurate record‑keeping and IT skills
- Ability to work independently and as part of a multidisciplinary team
- Experience of chronic disease clinics – desirable
- Ability to run nurse‑led clinics – desirable
- Cytology, spirometry, ECG skills – desirable
- Behaviour change skills – desirable
- Mentoring or supporting junior staff – desirable
