Social Worker
We are looking for a full time social worker to join our Gedling Local Mental Health Team (LMHTs).
Job responsibilities- Maintain an individual caseload and complete team responsibilities.
- Assess and analyze risk and ongoing risks to develop personalized risk and safety plans, balancing risk management, safeguarding, choice and self‑determination.
- Provide psychoeducation interventions and social work support to individuals experiencing social issues that impact their mental health.
- Work within a multidisciplinary team to improve mental health outcomes through understanding the social determinants of health.
- Represent the social work and social perspective in the team and lead on the trauma‑informed social model of mental health and wellbeing.
- Deliver evidence‑based practice informed by social work interventions, theories and values.
- Promote family and carers work within the multidisciplinary team and link in with the Family Intervention Team.
- Assist with safeguarding duties, including adult and children safeguarding responsibilities, and act as the safeguarding link role.
- Coordinate application of legislation (Care Act, Mental Capacity Act, Equality Act, Human Rights Act, Transforming Care, Mental Health Act and other relevant legislation).
- Work in partnership with external agencies and Social Care to support adults and carers with access to services and statutory social support.
- Model anti‑discriminatory and anti‑racist practice and challenge system impacts.
- Attend multidisciplinary team meetings and attend duty system as required.
- Provide training or share best practice within the team.
- Supervise colleagues when required.
- Deliver medication under guidance and relevant procedures, where occasionally required.
- Perform any other duties consistent with the role and part of the duty system.
- Current registration with Social Work England (SWE), maintaining professional competency in accordance with the social work standards and Professional Capabilities Framework.
- A full UK driving licence and vehicle for business use are required; reasonable adjustments are made for disabled individuals in line with the Equality Act 2010.

- Maintain an individual caseload and complete team responsibilities.
- Develop, coordinate and deliver personalized health and social needs assessments and care planning in collaboration with individuals, families, carers and support networks.
- Assess and analyze risk and ongoing risks, developing risk management and safeguarding plans.
- Plan and manage safeguarding in complex cases and support others in safeguarding skills.
- Interpret and apply current legislation and guidance.
- Work in an asset‑based, strengths‑focused approach, promoting self‑directed change.
- Collaborate with external agencies supporting adults and carers access to statutory and non‑statutory services.
- Manage complex risk, support others in risk management.
- Apply knowledge of harm and its impact on practice.
- Use evidence from multiple sources to inform decision‑making.
- Liaise with Social Care to ensure appropriate statutory support.
- Provide safeguarding expertise for adult and children safeguarding responsibilities.
- Promote family and carers work.
- Understand complex needs and mental health inequalities relevant to this group.
- Maintain expertise in legislation such as the Care Act, Mental Capacity Act, Equality Act, Human Rights Act, Transforming Care, Mental Health Act, and other relevant legislation.
- Critical knowledge of social work intervention theories and models for individuals, families, groups and communities.
- Specialised understanding of adult mental health models and approaches.
- Human rights approach with knowledge of statutory rights of individuals.
- Training in psychological and therapeutic approaches, able to integrate these into the social work role.
