Short Breaks Personalisation Officer – Children's Short Breaks Service | Whole Life Disability Team – Nottingham City Council

Are you passionate about improving the lives of disabled children, young people and their families? We are looking for a dedicated and compassionate Short Breaks Personalisation Officer to join our Children's Short Breaks Service within the Whole Life Disability Team.

This is an exciting opportunity to play a vital role in helping disabled children and young people access meaningful short break opportunities that promote independence, inclusion, wellbeing and positive outcomes.

About the Role

As a Short Breaks Personalisation Officer, you will work directly with disabled children, young people and their families to identify, develop and coordinate personalised short break support that meets individual needs and aspirations. You will be responsible for facilitating access to Short Breaks Services, supporting families through the application process, developing person‑centred support plans, and ensuring that services continue to deliver positive outcomes through regular reviews.

Key Responsibilities
  • Support disabled children, young people and their families to access short break opportunities and positive activities.
  • Develop creative and person‑centred support plans focused on individual outcomes and aspirations.
  • Process applications and referrals for Pathway 1 and Pathway 2 Short Breaks provision.
  • Provide information, guidance and advice regarding available short breaks, community activities and local support services.
  • Empower families to identify suitable support options, including the use of direct payments where appropriate.
  • Conduct annual reviews to ensure services remain effective and continue to meet identified needs.
  • Work collaboratively with families, education providers, health professionals and partner agencies.
  • Contribute to Education, Health and Care Plans (EHCPs) through written and verbal reports.
  • Identify and elevate safeguarding concerns and risks appropriately.
  • Assist in service development, data collection, quality improvement and user participation initiatives.
  • Represent the service at meetings, presentations and multi‑agency events.
About You
  • Has experience working with children and young people with disabilities and their families.
  • Understands person‑centred planning and outcome‑focused support.
  • Has knowledge of safeguarding practices and children's services.
  • Can build positive and effective relationships with families, professionals and community providers.
  • Is confident managing a varied caseload and prioritising work effectively.
  • Has excellent communication, organisation and problem‑solving skills.
  • Is passionate about empowering families and promoting independence, inclusion and choice.
  • Works well both independently and as part of a supportive team.
