Job Overview

This role sits within Nottingham West Primary Care Network, delivering safe, high‑quality nursing care predominantly to patients in care homes, with occasional work in GP practices or patients' own homes. You will be supported by the PCN Care Home Team, including registered general nurses, therapists, GPs and the wider management team, and will work within clear protocols to promote self‑care, monitor long‑term conditions and support prevention and screening activity.

Responsibilities

Work closely with care home residents, including those with frailty and complex needs, and be fully integrated into a supportive multidisciplinary team structure with regular supervision and oversight.

Support holistic care through weekly home rounds, assessments, care plan actions and participation in MDT discussions.

Carry out clinical tasks safely, including observations, venepuncture, urine testing, frailty risk assessments and supporting long‑term condition reviews.

Communicate effectively and document accurately using Systm One, escalating concerns appropriately and contributing to high‑quality, patient‑centred care.

Qualifications
  • Relevant Level 2 qualification (e.g., NVQ Level 2 in Health and Social Care or equivalent).
  • Experience in a patient‑facing or customer‑service environment with strong communication skills.
  • Ability to work independently within scope while contributing effectively to a multidisciplinary team.
