Short Breaks Personalisation Officer
Nottinghamshire, United Kingdom — Permanent, 18.5 hours per week, Hybrid Worker, starting salary £32,597 rising to £35,412 per annum (pro rata for part-time). The Short Breaks Team is based at Loxley House; the role is for hybrid working.
About the RoleWe have an exciting opportunity for one part time Short Breaks Personalisation Officer to join our busy team. The Officer supports families and disabled children/young people to access person‑centred short break provision.
Key Responsibilities- Enable access to Short Breaks Services for disabled children and young people in line with pathway one and two provision: process applications, screen for eligibility criteria, assess families' needs using application forms, professional letters, liaising with schools and other agencies and the Resource Allocation System (RAS); complete home visits and other necessary visits to capture holistic view and the child/young person’s voice;
identify level of support needed and indicative budget; complete annual reviews for Pathway Two support packages and process Pathway One re‑applications; follow up and answer short break enquiries. - Assist and enable service users to use their Personal Budget to meet social care and/or health care needs by helping them plan and source paid support and unpaid informal support from social networks and communities; complete person‑centred support plans and other mandatory documentation to request and refer to appropriate services in line with identified SEND outcomes.
The ideal candidate will have high standards of communication with vulnerable adults and children, be able to work in partnership with other professionals and agencies, and communicate effectively with disabled children and young people.
Essential Requirements- Knowledge of Personalisation and Person‑Centred ways of working.
- Knowledge of the SEND outcomes framework.
- Ability to work in a person‑centred way to develop individual care and support plans that meet identified outcomes, including conducting home visits.
- Experience managing and prioritising own workload, working independently, and recognising when support from the line manager is required.
- Experience maintaining electronic case files and administrative systems.
A DBS enhanced check for a regulated activity is required for this post.
