Field Care Supervisor

Job Description

Location: NG3, NG4, NG5, NG6, NG8

Pay Rate: £13.38 per hour plus 28p mileage

Shifts available: 07:00 - 22:00, four days per week + on‑call for additional payment of up to £250.00 per week

Shift Pattern: Week 1:
Monday, Tuesday, Thursday, Friday;
Week 2:
Monday, Friday, Saturday, Sunday

Responsibilities:

Carrying out initial risk and needs assessments in respect of individual service users, either in the service user’s home or, where required, in other settings such as hospitals or residential/nursing homes; working in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support.

Experience in carrying out initial risk and needs assessments and working collaboratively with service users, families, carers and healthcare professionals to deliver holistic care support.
