Waythrough is seeking a compassionate Support & Housing Management Coordinator to mobilise a hospital discharge service across County Durham. The role focuses on preventing homelessness by coordinating in-hospital and post-discharge support and helping individuals access and sustain accommodation.
You will work with partners to deliver person-centred plans, assessments and risk management. The position includes housing management duties and regular travel in the County Durham area.
