Personal Advisor – Leaving Care Team

Nottingham City Council is seeking a Personal Advisor – Leaving Care Team to provide high-quality, person-centred support to young people leaving care and help them successfully transition into independent adulthood.

The role involves managing a caseload of Children in Care and Care Leavers
, developing and reviewing personalised Pathway Plans
, and providing practical, emotional and developmental support to help young people build independence, resilience and long-term stability.

The successful candidate will work closely with young people, social workers and multi-agency partners to ensure care leavers receive the right support across areas including housing, education, employment, health and wellbeing.

• Manage a caseload of Children in Care and Care Leavers, ensuring statutory responsibilities are met.
• Develop, implement and regularly review individual Pathway Plans based on each young person’s needs, goals and circumstances.
• Build trusting relationships with young people, providing consistent support, guidance and advocacy.
• Support care leavers to develop independent living skills, including budgeting, tenancy management and accessing benefits.
• Help young people access and sustain suitable accommodation, education, training and employment opportunities.
