Community Care Officer

We are currently recruiting for a Community Care Officer to support a public client in Nottingham.

You will work as part of a professional team supporting vulnerable adults and their families, completing assessments, developing care plans and coordinating services to meet assessed needs.

Key responsibilities:
Undertake adult and carer assessments.
Develop and review person-centred care plans.
Manage allocated caseloads, including risk management.
Coordinate services and ongoing care provision.
Maintain accurate case records and reports.
Work closely with Social Workers, professionals and partner agencies.
Participate in multidisciplinary meetings and duty rotas.
Promote safeguarding, confidentiality and best practice.

Key requirements:
Experience working within Adult Social Care.
Strong assessment and case management skills.
Knowledge of safeguarding procedures and relevant legislation.
Excellent communication and report-writing skills.
Ability to work independently in a fast-paced environment.
Experience working collaboratively with professionals and partner agencies.

Contract Length: 3-6 months with expectation of an extension
Pay Rate: 22.52 per hour
Working pattern: 37 hours per week
