Base pay range: $45.00/hr - $55.00/hr
Bridge Home Health & Hospice is hiring a Certified Occupational Therapist Assistant contracted through the Organization. The COTA is responsible to the registered Occupational Therapist for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. The COTA performs only those services planned, delegated, and supervised by the registered Occupational Therapist.

- Participates in ongoing evaluation of patient’s functional status and home environment for hazards or barriers to more independent living, as delegated by the registered occupational therapist.
- Participates in teaching new skills or retraining patients in daily activities as appropriate.
- Maintains clinical records, notes, and reports to the registered occupational therapist any changes in the patient’s condition.
- Submits clinical documentation in accordance with Organization policy.
- Follows treatment program and goals established by the registered occupational therapist; documents patient’s response and progress.
- Maintains communication with other care team members to promote coordinated care and documents accordingly.
- Attends and contributes to in-services, case conferences, and other meetings as required.
- Identifies patient and family/caregiver needs for other home care services and assists with referrals, as appropriate.
- Participates in instructing patient’s family/caregiver and other health care personnel in the treatment regime as delegated by the registered occupational therapist.
- Is supervised by the registered occupational therapist no less than every thirty days; document ongoing communication, patient condition, response to services, and any plan-of-care changes.
- Adheres to attendance and reliability guidelines to ensure quality and consistent patient care.
Position
Qualifications:
- Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
- Certified with the National Certification Examination of the American Occupational Therapy Association.
- Licensed as an occupational therapy assistant by the California Board of Occupational Therapy.
- A minimum of one (1) year of clinical experience as an OTA.
- Good verbal and written communication and organization skills.
- Current CPR certification.
- Valid California Driver’s License with insured vehicle in good working order.
Physical Requirements:
- The ability to sit occasionally for 1-3 hours.
- The ability to frequently stand for 3-6 hours.
- The ability to occasionally drive/operate a vehicle for 1-3 hours.
- The ability to walk occasionally on level surfaces for 1-3 hours.
- The ability to walk intermittently on uneven surfaces for less than 1 hour.
- The ability to intermittently bend or climb for less than 1 hour.
- The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours.
- The ability to lift, carry, push, or pull up to 50 pounds intermittently in less than 1 hour.
