Responsibilities

• Perform physical examinations and preventive health measures within prescribed guidelines and instructions of a physician.
• Record physical findings; formulate plan and prognosis based on patient condition.
• Discuss case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribe or recommend drugs or other forms of treatment.
• Monitor and maintain progress of patients; report progress to supervising physician.
• Collaborate with hospice employees and volunteers as part of an inter‑disciplinary team (IDT).
• Preferred hospice experience.
• Assist in lifting and transferring patients as required.
• Maintain vision abilities: close vision, distance vision, and focus adjustment.
• Complete accredited Nurse Practitioner program.
• State licensure in good standing.
• Drive with reliable transportation, valid driver’s license, and auto insurance.

Qualifications

• AC credited Nurse Practitioner program completion.
• State licensure in good standing.
• Valid driver’s license and reliable transportation.
• Hospice experience preferred.
