Position: Regional MDS Registered Nurse (RN)

. Optalis Health & Rehabilitation is seeking a knowledgeable and driven Regional MDS RN to support our skilled nursing facilities throughout the Metro Detroit area. This key position plays a vital role in ensuring accuracy, compliance, and excellence in the Minimum Data Set (MDS) process while guiding and elevating facility‑level teams.

Position Overview

The Regional MDS RN oversees and supports the full MDS process across multiple locations, ensuring regulatory compliance and high‑quality documentation. This position functions as a partner, trainer, and expert resource to MDS Coordinators and interdisciplinary care teams throughout the region.

Key Responsibilities
  • Ensure timely and accurate of MDS assessments in accordance with State and Federal guidelines across assigned facilities.
  • Mentor, train, and provide ongoing support to facility MDS Coordinators and interdisciplinary teams.
  • Support facilities in appropriate reimbursement capture under PDPM and Medicaid Level systems.
  • Participate in quality and performance initiatives, including identifying trends impacting facility 5‑Star performance.
  • Communicate proactively with facility and regional leadership regarding concerns, barriers, and improvement opportunities.
Qualifications
  • Current Registered Nurse (RN) license in the State of Michigan.
  • Prior MDS experience is required; multi‑facility or regional experience is strongly preferred.
  • Strong working knowledge of Medicare/Medicaid reimbursement, PDPM, and Managed Care.
  • Exceptional interpersonal, communication, and relationship‑building skills.
  • Highly organized, accurate, and compliant with regulatory standards.
  • Proficiency with electronic documentation systems, Excel, Word, and MDS platforms.
  • Ability to educate, coach, and guide team members in both group and individual settings.
  • Commitment to quality outcomes and excellence in resident care standards.
