Warwickshire County Council is seeking a Social Worker to join our Hospital Social Care team at George Eliot Hospital in Nuneaton. You will provide a social work service to a high professional standard, manage a caseload, and travel across North and South Warwickshire as needed.
You will undertake robust assessments and care and support planning, communicating with customers, families, health partners and agencies. A driving licence is essential and weekend work is required on a rotating basis.
