GGz Centraal zoekt een enthousiaste begeleider om beschermende woonzorg in Nunspeet te versterken. Je begeleidt volwassenen met psychiatrische problematiek en dubbele diagnostiek en werkt aan activatie en maatschappelijke participatie.
Je werkt samen met het FACT-team en behandelaren, onderhoudt contacten met het sociaal netwerk en draagt bij aan individuele begeleidingsplannen. Onregelmatige diensten maken onderdeel uit van het rooster.
