La Fondation de La Côte à Nyon recherche un(e) professionnel(le) en soins infirmiers pour garantir des soins complets à domicile. Le candidat idéal a un Bachelor en soins infirmiers et une solide expérience en gestion de la communication et en travail d'équipe pluridisciplinaire.
Ce poste inclut la réalisation d'évaluations, l'élaboration des plans d'intervention, et une collaboration étroite avec les patients et leur entourage. Une bonne connaissance des outils comme Medlink est un plus. L'entrée en fonction est prévue dès le 1er septembre 2026.
