Overview

The Home Option is looking to hire a Registered Nurse (RN) or a Licensed Practical Nurse (LPN). The Home Option provides personalized medical care to help people live their best lives, including care for patients recovering from surgery or illness, coping with health challenges, or seeking end-of-life care. • Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility determination, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments vary depending on licensure.
• Obtain data and assess information from patient physical, psychological, social and spiritual factors that may impact patient and family needs; coordinate interventions with the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team and other agencies to coordinate optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician.
• Perform clinical pronouncement of death and provide support to the family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and timely care plans.

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation in accordance with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, reporting identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
• Use and/or prepare equipment and supplies effectively and safely, adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
• Provide health care instructions to the patient as appropriate per assessment and plan of care.

• RN
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience, preferably in intensive care, rehabilitation or medical-surgical nursing (preferred)
• LPN
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three (3) years of nursing experience; community/home health or medical/surgical experience is preferred
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
  • Home health experience preferred
  • Possess and maintain valid CPR certification
  • Must have reliable transportation, current driver’s license and appropriate automobile insurance
