What You'll Do:As the Patient Care Manager, you'll play a pivotal role in coordinating and overseeing patient care services while supporting and mentoring a skilled team of home health professionals. Your leadership ensures high-quality, compassionate care and compliance with all regulatory standards.
Key Responsibilities:Manage and coordinate the clinical care of patients in a home health setting
Supervise, mentor, and support a multidisciplinary team of clinicians
Ensure compliance with agency policies and home health regulations
Collaborate with physicians and referral sources to promote excellent care outcomes
Monitor quality assurance metrics and implement improvements as needed
Provide guidance in care planning, patient assessments, and documentation
Active RN license in Minnesota (required)
Minimum 1 year of home health experience (required)
2+ years of management or supervisory experience (preferred)
Strong leadership, communication, and problem-solving skills
Passion for patient-centered care and team development
