• Lead comprehensive nursing assessments and individualized care plans.
• Conduct OASIS visits and coordinate services across care settings.
• Collaborate with physicians and providers to maintain clinical continuity.
• Deliver education, emotional support, and transition planning for families.
• Ensure accurate documentation and regulatory compliance.

• MN RN license in good standing.
• 3+ years of clinical RN experience; home health preferred.
• Strong documentation, care planning, and assessment skills.
• Reliable vehicle and valid driver’s license.
