Job Summary

This position is an on‑call RN case manager with a 7

On/7

Off schedule: 5pm‑8am Monday‑Thursday and 5pm‑8am Friday‑Monday. The RN provides comprehensive assessment, care planning, medication management, and interdisciplinary collaboration for home‑based and facility hospice patients throughout the territory.

Responsibilities
  • Available to receive and respond to triage calls during assigned shifts, completing weekend admissions.
  • Complete a comprehensive initial assessment of patient and family to determine hospice needs for after‑hours calls, including a full physical assessment and health history.
  • Provide professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by:
    • Initiate and write the plan of care.
    • Re‑evaluate patient and family/caregiver needs.
    • Participate in revising the plan of care as necessary.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Use health assessment data to determine nursing diagnoses.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician within the physician’s plan of care.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Assist the patient with daily living activities and facilitate self‑sufficiency and optional comfort care.
  • After‑hours on‑call responsibilities include remaining available for all calls related to patient care and making all appropriate visits as necessary.
  • Remain with patient and/or family through a crisis period until resolved or relieved.
  • Maintain accurate clinical notes regarding patient’s condition and care given, recording pain/symptom management changes/outcomes as appropriate.
  • Communicate with the physician regarding patient needs, reporting changes in patient condition and obtaining physician orders as required.
  • Communicate with community health related persons to coordinate the care plan.
  • Teach patient and caregiver self‑care techniques, providing medication, diet, and other instructions as ordered by the physician and recognizing opportunities for health counseling.
  • Provide and maintain a safe environment for the patient.
  • Assist the patient and family/caregiver and other team members in providing continuity of care.
  • Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  • Attend interdisciplinary group meetings.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Assume responsibility for personal growth and development, maintaining and updating professional knowledge and skills through continuing education and in‑service classes.
  • Fulfill the obligation of requested and/or accepted case assignments.
  • Actively participate in quality assessment and performance improvement teams and activities.
Qualifications
  • A passion for hospice care and the ability to build relationships with patients and their families.
  • The ability to work efficiently with little direct supervision.
  • A valid driver’s license and an insured automobile that is in good working order.
  • 1‑2 years of recent acute care experience in an institutional setting (required). 1‑2 years of recent home care experience (preferred). We’re excited to train RN’s that are passionate about hospice care.
  • Graduation from an accredited school of nursing.
  • Understands hospice philosophy, is secure with issues of death/dying, and is comfortable providing specialized care to the terminally ill.
