Position: Clinical Pharmacist Specialist-Ambulatory Care, Per Diem, Days, 8hours

Overview

Summary: The Clinical Pharmacist Specialist is responsible for the provision of safe, effective, appropriate and cost‑efficient medication therapy management to patients in the area of Ambulatory Care. Under the direction of the Director of Pharmacy and site-based Medical Director, they assist in planning, organizing and monitoring the delivery of ambulatory care pharmacy services for the Alameda Health System.

An incumbent in the class of Clinical Pharmacist Specialist – Ambulatory Care coordinates the pharmacy services within AHS and its clients. This role requires advanced post‑graduate training or equivalent experience and a broad scope of pharmacotherapy knowledge for providing direct patient care as well as complex clinical services. In addition to administrative duties, the Clinical Pharmacist Specialist provides consultation to staff and clients.

Duties & Responsibilities
  • Work with the System Director of Pharmacy to coordinate, improve, expand and deliver ambulatory care pharmacy services within AHS.
  • Monitor patients in a specific unit or specialty service, including collecting patient data, interpreting drug‑related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.
  • Assess patient‑specific medical problems, prioritize based on urgency and severity, and identify preventive and health maintenance issues.
  • Evaluate patient‑specific drug therapy for appropriateness, effectiveness, and potential interactions or adverse effects. Document drug errors, adverse drug reactions, allergies, and patient adherence issues.
  • Design a comprehensive drug therapy and patient education plan for patient‑specific problems and justify recommendations based on pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal and evidence‑based information.
  • Under protocol, initiate, adjust, monitor, and recommend changes in drug therapy based on information from patient interviews, monitoring parameters, physical assessments related to drug therapy and adverse effect management, review of medication profiles and medical records. Document clinical interventions in the electronic health record in a timely and professional manner.
  • Collaborate with patients, caregivers and other healthcare professionals.
  • Develop and maintain the drug formulary for AHS; educate physicians and other providers regarding additions and revisions to the formulary, including non‑formulary review. Prepare drug monographs and cost analyses for presentation to committees, staff, physicians and other personnel. Maintain knowledge of current evidence‑based practice for applicable chronic disease states and serve as a drug information resource for team members.
  • Represent Clinical Pharmacy Services on committees, staff meetings, and professional meetings related to the pharmacy practice/specialty.
  • Coordinate continuing education activities for medical and nursing staff; orient new employees on pharmacy practices and procedures; contribute to pharmacy newsletters, participate in journal clubs, and provide in‑services to professional staff.
  • Serve as a preceptor to pharmacy residents, students and volunteers.
  • Minimum Qualifications
    • Education:

      Doctor of Pharmacy degree
    • Education:

      Completion of an American Society of Health-System Pharmacy accredited Postgraduate Year One (PGY1) Residency
    • Education:

      PGY1 or PGY2 Residency in Ambulatory Care is preferred
    • Minimum Experience:

      Three to five years of recent ambulatory care pharmacy experience preferred
    • Licenses/Certifications:

      Valid California pharmacist license
    • Licenses/Certifications:

      Basic Life Support (BLS) certification
    • Licenses/Certifications:

      Board certification (BCPS or BCACP) preferred
    • Licenses/Certifications:

      National Provider Identifier must be obtained upon hire

    Pay Range: $103.58/hr this pay range reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on factors including experience, education, skills, licensure and certifications, departmental equity, collective bargaining agreements, and organizational needs. Alameda Health System offers eligible positions a comprehensive benefits program.

    Additional Information: Note: SAN Clinical Pharmacist for primary care services (Anticoagulation, Refill, Diabetes, and Hypertension). This is to backfill PTO for full‑time clinical pharmacists in a variety of clinics. Coverage may include clinics away from the Wilma Chan Highland Hospital Campus at Freestanding Clinics (Eastmont, Hayward, and Newark). This is direct patient care in clinic, with some opportunity to work flexible hours if assigned to cover Refill services.
