We are currently seeking a compassionate, organized, and team-oriented Registered Nurse (RN) to join our Home Health team as a Full‑time or Part‑time (Per Diem) RN.
Position OverviewYou will evaluate patients' health, create and implement care plans, and track progress to maintain the highest standards of care. You will work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

- Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the interdisciplinary team.
- Administer treatments, monitor patient conditions, and report signs/symptoms of a change in patient condition to Physician, Case Manager, or Director of Nursing as needed.
- Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
- Supervise Licensed Vocational Nurses (LVNs) when assigned.
- Comply with all company policies and procedures to ensure high standards of care.
- Current RN License to practice in California (required).
- Experience in Home Health or Hospice care – 2 years required.
- Current CA Driver’s License.
- Able to travel within a 15–25 mile radius on a regular basis.
