Part‑time Registered Nurse Case Manager – Oakland Area

Anticipated Territory: Oakland Area

Schedule: Must be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time:
Saturday and Sunday plus 3 weekdays.

What you’ll do
  • Conduct Start of Care (SOC) admissions – OASIS
  • Conduct follow‑ups and evaluation visits as needed in designated territory
  • Make home visits to patients in designated territory
  • Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
  • Monitor clients’ conditions, reporting changes to the clinical team
  • Collaborate with physicians and agency staff to deliver the best care possible
  • Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

To be a success
  • Current and unrestricted CA RN license
  • Current BLS/CPR certification
  • Valid CA driver’s license
  • Experience conducting assessments and Start of Care (SOC) – OASIS is necessary
  • High level of clinical knowledge in home health care is highly preferred
  • Experience with high acuity patients is a plus
  • Experience working with an interdisciplinary team is highly desired
  • Case management fundamentals is highly desired
  • Previous experience with IV medication infusions is highly preferred
  • Proficiency initiating intravenous (IV) therapy is a plus
