Full-Time Salary: $105,000–$115,000 per year
Medical Hill Healthcare is seeking a compassionate, organized, and experienced Registered Nurse (RN) Case Manager to join our team. This position is responsible for coordinating comprehensive, resident-centered care and ensuring effective communication among residents, families, physicians, nursing staff, and interdisciplinary team members. The ideal candidate is an experienced RN with strong clinical judgment, excellent communication skills, and a passion for coordinating quality care and achieving positive outcomes.
Key Responsibilities- Coordinate and oversee individualized care plans for residents.
- Monitor resident progress and identify changes in condition or care needs.
- Collaborate with physicians, nursing staff, therapy, social services, and other members of the interdisciplinary team.
- Conduct assessments and assist with developing and updating care plans.
- Facilitate communication with residents and families regarding care needs and services.
- Monitor hospitalizations, transfers, discharges, and transitions of care.
- Assist with identifying barriers to care and developing appropriate interventions.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in care conferences and interdisciplinary meetings.
- Support quality improvement and regulatory compliance initiatives.
- Promote continuity of care and advocate for resident needs.
- Perform other duties as assigned.
- Current and active California Registered Nurse (RN) license.
- Previous nursing experience required; case management, care coordination, or skilled nursing experience preferred.
- Strong clinical assessment and critical-thinking skills.
- Excellent written and verbal communication skills.
- Strong organizational and time-management abilities.
- Ability to work effectively with residents, families, and interdisciplinary teams.
- Knowledge of care planning, documentation, and regulatory requirements.
- Professional, dependable, and compassionate approach to resident care.
