Position: CORE Reentry Coach - Alameda County

Overview

Center of Reentry Excellence(CORE) Reentry Coach

The Reentry Coach performs a wide range of critical roles to ensure the successful operation of a reentry focused multi-service center. The Coach is responsible for coordinating the Center’s delivery of critical reentry services to formerly incarcerated residents of Alameda County. In collaboration with Center staff and partners, the Reentry Coach fosters client engagement, service planning, performance management, and client retention support.

By engaging clients, partners, and other stakeholders in the community, the Reentry Coach is able to develop successful partnerships and processes that effectively promote success for the Center’s clients and partners alike. Frequently the Coach serves as a “first contact” for clients as they begin to engage with services from the Center, and may serve as a “problem solver” to help improve service delivery for clients or partners.

The Reentry Coach plays an important role in helping the Site Manager set and maintain the Center’s welcoming culture of acceptance, inclusion, restoration and collective excellence. This Reentry Coach reports to the Lead Reentry Coach.
Working evenings and on weekends may be required for this role. Alameda County residents and formerly incarcerated individuals are strongly encouraged to apply.

  • Partnership support: Serve as primary point of contact for CORE partners in their direct-service work with clients, supporting smooth and coordinated development and implementation of client service plans; help address day-to-day logistical or collaborative challenges experienced by partners (either between partners or between a partner & client); help partners make appropriate referrals to needed services (both on-site and off-site); as needed, coordinate and participate in collaborative case conference meetings.
  • Data gathering and use
    :
Gather and enter appropriate client data at each “touch point” along a client’s journey, and support partners in the collection and reporting of information related to shared service delivery objectives.
  • Outreach
    :
