Community Nursing Palliative Care; RN

Part‑time: 2 shifts per week, 1:3 weekends plus an On‑call rotation.

We are seeking a compassionate and experienced Community Visiting Nurse specializing in Palliative Care to provide high‑quality, patient‑centered care to individuals with serious illnesses in their homes, focusing on symptom management, pain relief, and emotional support.

Location:

Halton area (Oakville focus). Registration with the CNO required.

Pay Rate: $34.76 – $37.64 per visit.

• Provide comprehensive nursing care to palliative patients in their homes within the Halton area.
• Organize the day, set priorities based on continuity of care, type of care to be given and the client’s health needs.
• Develop, implement, evaluate, and modify the plan of care outlining strategies, orders and actions to achieve expected/desired outcomes.
• Assess the health status of the client.
• Administer IV therapy including central line and peripheral line maintenance.
• Administer medications topically through natural orifices, through tubes and by injections.
• Handle wound care, acute and chronic, for certain types and stages of wounds.
• Ensure the laptop is synchronized with the network at least twice a day, at the beginning and end of the shift, to receive information and update client records.
• Attend team/POD meetings as required.

• Minimum 1 to 2 years of clinical practice in a community environment (preferred).
• Minimum 1 to 2 years of nursing experience preferably in palliative care, hospice, or home health.
• Current registration with the CNO.
• Certification in Palliative or hospice care (preferred).
• Current CPR certificate.
• Excellent interpersonal, communication and teaching skills.
• Valid driver’s license and access to a vehicle.
• Legally eligible to work in Canada.
