Position: Community Visiting Nurse Evenings with Acclaim Health
Become a Community Visiting Nurse with Acclaim Health, working evenings from 4 PM. Deliver essential healthcare services directly at clients' homes in Halton.

This role requires a dedicated Registered Practical Nurse, committed to providing high-quality care including medication management and palliative support. You'll adapt to various client needs while supporting their independence and well-being. This position involves a minimum of two evening shifts per week, plus 1:3 weekends.

Key Responsibilities:

• Deliver nursing services within the community and clinics

• Organize daily care based on client needs

• Assess health status and document changes

• Administer medications and perform wound care

• Ensure effective communication within the care team

Requirements:

• Minimum 1-2 years of community nursing experience

• Registration with the CNO and current CPR

• Excellent interpersonal and teaching abilities

• Preferred experience in palliative care

• Valid driver’s license required
