What you will do:

• Conduct assessments and develop personalised care packages for adults and young people with learning disabilities using Personal Outcome Planning tools.
• Implement and consistently review these care packages to ensure they meet the evolving needs of service users and their carers.
• Liaise with Community Care Managers and service providers to commission services that meet assessed needs.
• Utilise the department’s Eclipse information system for record-keeping and data entry.
• Participate in supervision and training opportunities for professional development.
• Be part of the on-call rota, providing crucial out-of-hours support as needed.
